Poorly fitting Dental Crowns

This gentleman whose dental photograph is given here came to me with complaint of food impaction in between the “crowned”(caps) teeth and the natural tooth ie the second premolar.

Oral examination revealed that the tissues adjoining the artificial crowns are chronically inflammed. There is profuse bleeding on slight provocation with a dental probe.

A dental radiograph revealed that there is periodontal breakdown in between the artificial crowns and the natural tooth. And as a result of perpetual food impaction there has also developed a cavity on the natural tooth.

The natural question is why did it happen? – The whole trouble has been caused by the poorly fitting crown. The patient has got his root canal treatment done and subsequent capping from some dentist. It is not ethical for me to comment on the quality of dental work – so I’m limiting myself to the dental complaint of the patient.

The crowns in this case have such a poor fit that these invite food impaction thereby leading to stagnation, chronic infection, bleeding and resultant periodontal breakdown.

I commonly encounter such patients where the artificial crowns are the source of a lot of problem. Please don’t take otherwise – there is nothing wrong with the treatment involving dental crowns. But if these are not made properly, these are a headache for both — patient and the dentist .Why dentist? – after all patient will be going to him only for any dental complaints related to the treatment provided by a dentist.

So, lesson of the story is neither the patient should insist on a particular type of treatment nor the dentist should be in a hurry in proper case selection. The right candidates for receiving treatment with fixed appliances need to be carefully selected —otherwise, later on the patients never share the blame regarding there poor bone health etc . By the way,why should they ? Hurriedly, let me add such problems mostly crop up when the treatment has been rendered by unqualified doctors called quacks who are in abundance everywhere.
